Community Hospice and Palliative Medicine Fellowship Clinical Mentor (Instructor Adjoint) Community Hospice and Palliative Medicine Fellowship Clinical Mentor (Instructor Adjoint) - 40734 Faculty Description University of Colorado Anschutz Department: Medicine- Division of General Internal Medicine Job Title: Community Hospice and Palliative Medicine Fellowship Clinical Mentor (Instructor Adjoint) Position #00851592 - Requisition #40734Job Summary: The CHPM fellowship pathway is a part time 2-year virtual clinical fellowship in conjunction with a 2-year Master of Science in Palliative Care. The MSPC is administered through the University of Colorado Anschutz graduate education programs and teaches a diversity of palliative care providers-physicians, nurses (including BSN and advanced practice nurses), physician assistants, pharmacists, social workers, spiritual care providers, psychologists, therapists, counselors and ethicists-to be Palliative Care Community Specialists. Graduates provide high-quality palliative consultation and care to patients and families living with serious and life-threatening illness. The MSPC is foundational for the University of Colorado Community Hospice and Palliative Medicine (CHPM) Fellowship for physicians who wish to become board-certified practitioners in palliative care and hospice. Physicians do their academic coursework in the CHPM and MSPC virtually, while the clinical portion of the CHPM is done in fellows' own geographic location either with or without direct observation of their clinical interactions with patients. Clinical reasoning and skill are assessed and documented in patient logs that include patient write-ups, journal clubs, asynchronous self-taped videos of communication skills and documentation of all clinical requirements.
